The Systems Navigator provides intake services for new clients, and provides a comprehensive assessment of their strengths, capacities and needs based on the social determinants of health. They will assist clients in navigating the broader health care system while also establishing and maintaining a strong relationship with community partners.
This position offers a unique opportunity to make a difference in the lives of newcomers and refugees, and those significantly marginalized by poverty, addiction, mental health, and housing issues. This role will work with our newcomer clinic, supporting newcomers to Canada along the continuum of care within a health promotion framework and liaising with interdisciplinary team members and external care providers to facilitate the best plan of care for clients.

THE COMPENSATION PACKAGE
- A salary range of $51,000 - $55,000
- Monday to Friday - no weekends!
- A Psychologically Safe and Inclusive Workplace
- 100% employer-paid health & dental package, including up to $2,000/benefit year for mental health, $1,500 for massage therapy
- Healthcare of Ontario Pension Plan (HOOPP)

REQUIRED QUALIFICATIONS
- Completion of postsecondary education in social services or related health care discipline
- Experience in community, health and/or social services working with highly vulnerable people
- Ability to assess and evaluate a client’s health and social needs, as expressed by the client
- Understanding of the social determinants of health & health promotion
- Willingness to work with people of diverse backgrounds with an inclusive and anti-oppressive lens
EXTRAS TO TELL US ABOUT
